Ovarian cysts are fluid-filled sacs that can form on or inside an ovary. Many are functional cysts related to ovulation and go away on their own. Others need monitoring or treatment, especially if they are large, persistent, complex, or causing symptoms.

Symptoms to watch

An ovarian cyst may cause one-sided pelvic pain, bloating, pressure, pain with sex, changes in bleeding, or discomfort that comes and goes with your cycle. Some patients feel fullness after eating only a small amount or notice urinary frequency from pelvic pressure.

Sudden severe pelvic pain, pain with fever or vomiting, fainting, shoulder pain, or signs of shock require urgent evaluation. These symptoms can occur with cyst rupture, bleeding, or ovarian torsion.

How cysts are evaluated

Your provider may perform a pelvic exam and order an ultrasound to look at cyst size and appearance. Depending on your age, symptoms, pregnancy status, and ultrasound findings, follow-up imaging or blood tests may be recommended.

Not every cyst needs surgery. Many can be monitored. Treatment may be considered if a cyst is painful, growing, persistent, suspicious in appearance, or affecting fertility or daily life.

Follow-up helps distinguish a normal ovulation-related cyst from a cyst that needs more attention. It also gives you a clear plan for what symptoms should prompt a call.
