Signs your flow may be heavier than expected
Consider scheduling an appointment if you soak through a pad or tampon every hour, pass clots larger than a quarter, bleed longer than a week, feel lightheaded during your period, or avoid normal activities because of your flow. Fatigue, shortness of breath, headaches, and craving ice can also point to iron deficiency from blood loss.
Heavy bleeding should also be evaluated promptly after menopause, between regular periods, after sex, or with new pelvic pain.
What an evaluation may include
Your visit usually starts with a detailed cycle history. Your provider may ask about bleeding patterns, pain, medications, pregnancy history, birth control, and family history. Depending on your symptoms, testing can include a pelvic exam, pregnancy test, blood count, thyroid testing, ultrasound, Pap or HPV testing when appropriate, and sometimes endometrial sampling.
The point is not to order every test for every patient. It is to choose the right next step based on age, risk factors, and how the bleeding is affecting your life.
Treatment depends on the cause
Some patients improve with anti-inflammatory medication, iron support, or hormonal birth control. Others benefit from a hormonal IUD, medication that reduces bleeding, treatment for fibroids or polyps, or an in-office procedure such as hysteroscopy or endometrial ablation when appropriate.
